Abstract

Electronic Health Records (EHRs) hold private and sensitive information. Patient healthcare data is not secure under the existing centralized system, which is a major cause for concern. Blockchain technology can safeguard the privacy of private medical records. This research aims to design and implement EHR-Chain, a novel privacy-preserving framework that integrates Hyperledger Fabric and off-chain storage to enable secure, scalable, and revocable sharing of electronic health records with enhanced privacy-preserving features. This framework designed to meet Health Level Seven International (HL7) Fast Healthcare Interoperability Resource (FHIR) which provides global standards for the electronic exchange of healthcare information. Key achievements include robust patient privacy protection through HSM-secured ECDSA P-256 cryptographic identities and AES-256-GCM encryption, with comprehensive audit capabilities enabled by the immutable ledger. The system maintains strong performance 80–150 TPS with sub-second latency across varying network sizes while efficiently handling large medical files through IPFS integration.
