Abstract

Abstract The healthcare sector in Kerala is widely recognized for its strong public health indicators and a well-integrated public–private service delivery system. Within this context, Thiruvananthapuram (Trivandrum), as the administrative capital and a major healthcare hub, presents a unique setting to examine leadership and organizational outcomes in hospitals. This study investigates the influence of transformational leadership on job satisfaction, work engagement, and organizational commitment among healthcare administrators working in public and private hospitals in Trivandrum district. Using a quantitative research design, data were collected from healthcare administrative professionals through structured questionnaires. Statistical analyses were employed to examine the relationships among leadership style, psychological empowerment, and key employee outcomes. The findings indicate that transformational leadership has a significant positive impact on job satisfaction and work engagement, which in turn enhance organizational commitment. The study highlights the critical role of leadership practices in fostering motivated, engaged, and committed administrative personnel, thereby contributing to organizational effectiveness and resilience in a complex healthcare environment. The results offer valuable insights for policymakers, hospital management, and leadership development initiatives aimed at strengthening healthcare administration in mixed health systems.
